Supplier Information & Tax Forms

MSU Procurement will use information provided by your organization to create a PO supplier record in our Financial System. For security purposes, suppliers are required to obtain an IRS Tax EIN Number (as opposed to a social security number) to do business with MSU.

Required forms

• Supplier Information (.pdf)
• Domestic suppliers: W-9 (.pdf)
• Foreign suppliers: appropriate W-8 form
